Abstract

This book offers a highly informative guide to forceps devices and delivery techniques. Extensive figures and animations offer readers vivid insights into the fetus station, pelvis, and inserted forceps. The book covers normal presentation and abnormal rotation, presentation and attitudes, and especially focuses on the assessment of fetal descent. It discusses the problem with conventional fetal station, and explains the revised one based on the trapezoidal plane. Instrumental delivery is chosen in cases of worrisome fetal status, protracted labor, or maternal fatigue. Forceps can provides a greater pulling power than vacuum extraction, and the fetus can be delivered in a short time, though this technique calls for experience and technical proficiency. The book argues that in order to ensure safe and reliable forceps delivery, assessment of the fetal station and the site of the largest fetal head circumference is critical. The book offers a new and indispensable reference guide for all obstetricians, not only for residents, but also for all experienced professionals involved in the training of beginners, and even for those using vacuum extraction who would like to assess the fetus more objective and practically.

Abstract

Over the past decade there have been major advances in the field of regenerative medicine with the promise to bring to reality, cures for debilitating diseases such as diabetes, heart failure, and Parkinson’s disease. Cellular products from a variety of sources are being evaluated for their ability to replace damaged tissue. Fetal tissues consist of stem cells and progenitor cells which have undergone initial commitment with varying states of differentiation. Stem cells from fetal tissues may also have a greater proliferative potential than their adult counterparts. In addition, fetal derived stem and progenitor cells are immunologically naive and some sources of fetal cells, eg cord blood, have been shown to be capable of crossing greater HLA mismatching resulting in less rejection and decreased immune mediated toxicities. Given the increasing focus on HES and advances in our basic knowledge of regenerative medicine it is an appropriate time to review the biology and use of fetal tissues. Human Fetal Tissue Transplantation is a timely publication that provides details of many aspects of the potential use of fetal tissues for therapeutic applications. As many tissues are wasted on a daily basis it appropriate to raise discussion on how to maximize access to discard tissue and at the same time engage in discussion of the ethics associated with fetal tissue procurement and clinical use.

Abstract

This book is a practical, evidence-based resource covering thyroid disease and its effects on reproduction and fertility, discussing thyroid function and dysfunction - hypothyroidism and hyperthyroidism, thyroid nodules and cancer - during pregnancy as well as its effects on the fetus. It begins with a review of the anatomy and function of the thyroid and the biosynthesis and physiology of thyroid hormones, as well as the pathophysiology of thyroid dysfunction. The chapters that follow discuss thyroid function and dysfunction (e.g., hypothyroidism and hyperthyroidism) during pregnancy and its effects on fertility as well as the developing fetus. The management of thyroid nodules and cancer, the role of thyroid dysfunction in infertility, the role of thyroid autoimmunity and miscarriage, and postpartum thyroiditis are presented in detail in the concluding chapters. Clinical cases and outcomes bookend each chapter, demonstrating current management strategies for a well-rounded presentation. Filling a clear gap in the literature usually covered only briefly in larger texts on the thyroid, Thyroid Disease and Reproduction: A Clinical Guide to Diagnosis and Management is a unique resource for reproductive endocrinologists, obstetrician/gynecologists, and residents and students in these clinical areas. .

Abstract

Prenatal life is the period of maximal development in animals, and it is well recognised that factors that alter development can have profound effects on the embryonic, fetal and postnatal animal. Scientists involved in research on livestock productivity have for decades studied postnatal consequences of fetal development on productivity. Recently, however, there has been a surge in interest in how to manage prenatal development to enhance livestock health and productivity. This has occurred largely due to the studies that show human health in later life can be influenced by events during prenatal life, and establishment of the Fetal Origins and the Thrifty Phenotype Hypotheses. This book, Managing the Prenatal Environment to Enhance Livestock Productivity reviews phenotypic consequences of prenatal development, and provides details of mechanisms that underpin these effects in ruminants, pigs and poultry. The chapters have been divided into three parts: Quantification of prenatal effects on postnatal productivity, mechanistic bases of postnatal consequences of prenatal development and regulators of fetal and neonatal nutrient supply. Managing the Prenatal Environment to Enhance Livestock Productivity is a reference from which future research to improve the level of understanding and capacity to enhance productivity, health and efficiency of livestock in developing and developed countries will evolve. It is particularly timely given the development of molecular technologies that are providing new insight into regulation and consequences of growth and development of the embryo, fetus and neonate.

Abstract

The book Gamete and Embryo-fetal Origins of Adult Diseases introduces various diseases resulting from the abnormal gametogenesis and embryo development, which manifests as growth retardation, birth defects, or increased susceptibility to chronic metabolic diseases such as diabetes, cardiovascular disease and cancer in childhood and adult life, even fertility disorders and the risk of transgenerational transmission. Six common kinds of these diseases are discussed in separate chapters. The authors explore the connections between these diseases and epigenetic reprogramming, rapid cell differentiation and organ formation and environmental influences, including assisted reproductive technology and adverse intrauterine environments. With a summary of findings on the causes and progression of adult diseases at the phase of gametogenesis and embryo development, this book provides insights into the pathogenesis of disease and aids in the treatment and prevention of disease, meeting the requirement for improving the quality of the newborn population, and effectively preventing and curing major diseases at an early stage. This book offers new perspectives and will be an enlightening resource for obstetricians, paediatricians, epidemiologists, endocrinologists and sanitarians. Editor He-Feng Huang, M.D., is Professor and President of Women’s Hospital, School of Medicine, Zhejiang University, China. Editor Jian-Zhong Sheng, Ph.D., is Professor at the Department of Pathology & Pathophysiology, School of Medicine, Zhejiang University, China.
